A Comparative Study on concerns of Primiparas during the First and Fourth Postpartum Week


Abstract
Many physical and psychological alteration take place immediately after birth, precipitating some worry or anxiety in a new mother. However, some of changes may be of interest to her.
A mother concerns are an indication of her needs at a given time. By assessing each mother, individual needs can be met by teaching, home visiting and other means.
In order to identify the major concerns and their changes during pospartum period, and to compare the concerns of two groups, 43 primiparas who had a postpartum education and home-visiting and 47 primiparas who had not, completed a questionnaire with concerns related to self, infant and husband, on the first postpartum week, and 38 of them on the fourth postpartum week.
The collected data were analyzed by the SAS program using frequency, X2-test, t-test, ANOVA, pared t-test.
The results were as follows:
1. The aress of concern showed significant difference between the first and fourth postpartum week were as follows; `discomfort from episiotomy¢¥ `fatigue and lack of sleeping¢¥, `discomfort from constipation & hemorrhoid¢¥, `infant physical care¢¥, `comfortable handling¢¥, ¢¥recognizing illness¢¥, `jaundice¢¥, `baby safety¢¥, change of sleeping pattern¢¥, and `sexual relations¢¥. Among above them, concerns about ¢¥fatigue and lack of sleeping¢¥, ¢¥discomfort from constipation & hemorrhoid¢¥, and ¢¥sexual relations¢¥ were more worry on the first than the fourth postpartum week, and concerns related to infant decreased.
2. The frequency of concerns related to infant was the most highest both two groups on the first postpartum week (both more than 95.2 %), ¢¥discomfort from episiotomy¢¥ (more than 40 %), ¢¥infant physical care¢¥ ¢¥comfortable handling¢¥ ¢¥recognizing baby illness¢¥, ¢¥jaundice¢¥, ¢¥baby safety¢¥ (more than 30 %) in worry or anxiety concerns, ¢¥return of menses¢¥, ¢¥taking body temperature¢¥, ¢¥interpreting behavior of infnat¢¥, ¢¥process of growth and development¢¥ (more than 3.6.2 %) in interest concerns.
3. There were significant differences in 18 items of total of 36 items between two groups, mothers who had a postpartum education and home visiting expressed less worry or anxiety, more no problem or no concerns than control group.
4. The significant variables among the sociodemographic variables influencing concerns on the first postpartum week were education level and house environment. (t= -2.84, p < 0.01; t= -2.28, p < 0.05).
5. The first rank of source of help was mother-in-law (or she¢¥s mother), the secondrank, nurse on the first week, but booklet, neighbor, husband and next rank, nurse showed on the fourth postpartum week.
